We are currently seeking a Case Manager who shares our values and is committed to delivering exceptional, person-centred care coordination to our clients.

About the Role

As a Case Manager, you'll be responsible for managing a portfolio of private and government-funded clients across a variety of settings. You will conduct client intake assessments, develop and review care plans, coordinate services, manage budgets, and ensure the highest standard of care is provided - all while working closely with clients, families, and care teams.

You'll be supported by a collaborative, professional team who are just as passionate about people and purpose as you are.


What You'll Do

  • Conduct holistic assessments and develop individualised, goal-based care plans

  • Coordinate and delegate care services to support client wellbeing

  • Ensure client services are delivered within budget and according to government funding guidelines

  • Build genuine, trusting relationships with clients, families and care teams

  • Monitor client health and wellbeing, and respond to changing needs or risks

  • Support clients in making informed decisions, promoting independence and reablement

  • Maintain audit-ready client files and contribute to continuous improvement


What You'll Bring

  • Experience in community or home-based care

  • Strong case management and care coordination skills

  • Excellent interpersonal and communication abilities

  • Confidence managing client budgets and service plans

  • Attention to detail and a proactive, solution-focused mindset

  • Ability to build meaningful, respectful relationships

  • A current driver's licence and willingness to travel to client homes


Preferred Qualifications (not all required)

  • Diploma of Nursing or related Allied Health qualification

  • Certificate III or IV in Aged Care, Disability, Community Services or Health Assistance

  • Diploma in Community Services (Case Management), Ageing Studies or related field
