Do you have a background in aged care or health - and a heart for helping families navigate life's most delicate transitions?
We're looking for a compassionate, capable, and proactive professional to join our team as an Aged Care Placement Consultant - someone who understands the system and the emotions behind the decisions. If you've worked in admissions, case management, or care coordination, and want to move into a more autonomous, client-focused role, this could be the meaningful change you've been looking for.
This is more than a job - it's a chance to walk alongside older Australians and their families during one of the most challenging and personal decisions they'll make.
You'll support your own caseload from initial inquiry through to aged care placement, acting as a trusted advisor and advocate. It's a varied role combining clinical insight, emotional intelligence, and operational coordination.
Offer expert guidance and reassurance to families exploring aged care options
Match clients to suitable homes based on care needs, financials, and preferences
Liaise with hospitals, discharge planners, and providers to ensure smooth transitions
Arrange and accompany families on facility tours where appropriate
Assist with paperwork including Centrelink, ACAT, and admissions documentation
Keep clear and up-to-date client records in our CRM system
Contribute to a collaborative, values-aligned team
You're warm, practical, and deeply motivated to help people. You're also:
Experienced in aged care, nursing, allied health, admissions, or case management
Comfortable explaining care fees and systems - or willing to learn quickly
An excellent communicator who builds trust easily
Highly organised, able to juggle priorities with calm efficiency
Ready for a role where what you do really matters
